We’re excited to welcome you to Tucker Dental Excellence and help make your first visit comfortable and stress-free.
To help your appointment run smoothly, we ask all new patients to arrive 15 minutes early to complete registration and necessary paperwork before your scheduled appointment time.
To save time during check-in, you can also download and complete your forms before your visit. Simply print the paperwork and bring it with you to your appointment.
Please remember to bring your photo ID, insurance information (if applicable), and any completed forms with you.
